When applying for a passport please bring the following things with you:

  • Certified copy of applicant's Birth Certificate, Birth Abroad or Original Naturalization Certificate if they were not born in the US. Copies of Certified copies, Hospital Birth Certificates and Birth Registrations/abstracts are not acceptable. You can obtain a Certified copy of your Birth Certificate from Vital Records in the State from which you were born.

  • One Passport Photo. These need to be specific passport photos, please visit the Passport Agency Website for details.

  • Driver's License or State ID. If applicant is under 16 years of age, we require both Parents Identification. If applicant is older than 16 but younger than 18 we will require one parent ID, and one from applicant. NOTE: State Identification card must have picture and be current, no expired ID's or temporary ID's will be accepted.

  • Any other required documentation needed for the application process.

Prior to the appointment time, please have all application forms filled out completely and not signed, we have to WITNESS your signature on the application.

The Passport fees are two-part

Passport Book: Normal processing 4-6 weeks   Passport Card: Normal processing 4-6 weeks

Expedite passport fee - $60.00 (processing time 2-3 weeks)

Forms to be used: All passport forms can be found at travel.state.gov or in our office.

First Passport Application, or expired passport is over 15 years since first issued, or current or expired passport was issued when applicant was under 16 years of age.

Form DS-11

Renewing expired passport that was issued to applicant at age 16 or older, and passport was issued 15 years or less.

Form DS-82

All persons applying for passport must appear in person, if applicant is under 16 years of age, both parents are required. if applicant is older than 16 but younger than18 only one parent is required with applicant.

If one parent cannot be present, the parent who cannot attend must have a notarized Parental Consent Affidavit in place of the parent. you can find this form on the passport agency website.

Payments:

We cannot accept Cash or Debit/Credit cards for Passport fees at this time, however we can accept cash for the Clerk's Processing fee portion.

When paying with check or money order, Please bring more than one check or money order according to the fees stated above.

The Clerk's processing fee can be combined, however, each application must have the passport fee attached individually, we cannot combine the fees into one check for multiple applications.
